When applying for the program, the following documents must be provided:

1. 3×4 sized 6 photo;
2. 086-У form medical certificate (original);
3. Fluorography snap (original);
4. 063 vaccination card (copy);
5. pre-conscription paper or military registration card for male applicants (copy);
6. ID card (original and copy);
7. Birth certificate and the ID of one of the parent (original or copy) for minors;
